The Mutima Care model tackles the ‘revolving door’ of hospital readmissions. Led by experts in Implementation Science, we apply academic rigour to the reality of home care.
We know the pressure is double-sided: delays in getting home, and the preventable crises that send people back to hospital. Our goal is to stop both. Subject to registration, we are building a service designed to identify early warning signs; like infections or fall risks so we can act before they become emergencies.

We believe care should be proactive, not just reactive. We are establishing digital systems to monitor vital health trends, such as hydration levels, skin integrity, and mobility.
Instead of waiting for a crisis, this data-led approach is intended to allow our team to spot small changes early. By intervening fast, we aim to keep your loved one safe, stable, and out of hospital.
